Clinical Study of Stroke

Abstract
Stroke is one of the most frequent diseases of the elderly. and major cause of disability in the elderly population. But in many ways not contributory to our understanding of their medical and maintenance needs. For this reason, value of rehabilitation to improve the patient after stroke should be chailanged and to be developed a method of assessing outcome of treatment that would enable professionals to respond better to the needs of stroke patients.
This paper is to examine the rehabilitation outcome of 314 adult stroke conducted at the Department of Rehabilitation Medicine, Korea University Hospital in using the same criteria of evaluating the patient¢¥s functional status which were conducted by Stroke Adult Study of North America(SAS) in 1979.
1) The subjects comprised 314 cases of CVA, ranging from 26 to 85 years (mean-58.6 yrs) and included 190 males and 124 females. Among these patients, 115 were hemorrhage in origin and the rest were thrombotic or others.
2) As for the duration of the onset of the stroke to admiss-on, over 49.5% of the patients were admitted within 30 days after the onset of the stroke, on the other hand 70% of SAS patients were admitted within one month.
3) 92.7% of the patients were discharged within 2 months as compared to SAS in which 80¡Æ0 of the stroke patients were discharged within 2 months of their hospital stay.
4) Visual perceptive disturbances were found in 76 out of 124 tested subjects and left side paretic patients performed worse than right side patients on visual perception tests, and improved significantly in right side paretic patients at the time of discharge.
5) 53% of 252 test eligible patient had communication impairment, 19% among them were aphasic and 77.50/¢¥ of aphasic patients were right side paretic.
6) The patient¢¥s functional status was evaluated with 132 subject by Barthel index, an ADL functional score, at the time of admission and at the time of discharge, it revealed that 23.28 and 49.23 respectively. It was found that the Barthel independent score was in 7.6¡Æ0 at the time of admission and 46.2¢¥,0 at the time of discharge compared with 18.2`.0 and 65.7¡Æ-0 respectively in SAS.
